Fertility Network USA is a United States fertility clinic directory. The directory displays clinical outcome data sourced from the CDC National ART Surveillance System (NASS), 2022 Annual Report. The site is operated independently and is not affiliated with the CDC, SART, or any government agency.

2. Information We Collect

2.1 Information You Provide (Clinic Listing Form)

When a clinic or practice submits details through the List Your Clinic form, we collect:

• Clinic or practice name
• Physical address and city
• Clinic website URL
• Contact person name and role
• Email address
• Phone number
• Treatments offered
• Clinical details (CDC live birth rate, annual cycles, medical director name, SART membership)
• Listing tier preference (Free, Featured, or Sponsored)
• Any additional message entered in the free-text field

This information is submitted voluntarily. You are not required to use the form or submit your clinic.

2.2 Information Collected Automatically

Fertility Network USA is hosted on Cloudflare Pages. Cloudflare may collect server-side request logs including IP addresses, browser user agent strings, and request timestamps as part of its standard infrastructure operation. This data is governed by Cloudflare’s Privacy Policy.

Fertility Network USA does not currently set first-party cookies, use tracking pixels, or run third-party analytics scripts on any page.

2.3 Information We Do Not Collect

• We do not collect patient personal information. The directory is for clinic research only.
• We do not collect payment card details directly. Payment processing (if applicable) is handled by a third-party processor.
• We do not collect Social Security Numbers, date of birth, or government identifiers.

3. How We Use Your Information

Information submitted through the clinic listing form is used for the following purposes:

• Directory listing: To create or update a clinic profile on fertilitynetworkusa.com
• Verification: To confirm clinic details against SART records and public databases before publishing
• Communication: To respond to your submission, confirm listing activation, or discuss paid placement options
• Directory operations: To maintain accurate and current clinic contact information in the directory

We do not use submitted information for unsolicited marketing, third-party advertising targeting, or sale to data brokers.

4. How Your Information Is Stored

Clinic listing form submissions are stored in a Google Sheets spreadsheet owned and controlled by Fertility Network USA. Data is transmitted from the website to Google Sheets via a Google Apps Script Web App. This transmission is encrypted using HTTPS.

Google Sheets is a service provided by Google LLC. Data stored in Google Sheets is subject to Google’s Privacy Policy. Google does not have the right to use your data for its own purposes under Google Workspace service terms.

We retain clinic submission data for as long as the clinic listing remains active or until deletion is requested. Submissions for clinics that are not approved for listing are retained for up to 90 days and then deleted.

5. Information Sharing and Disclosure

Fertility Network USA does not sell, rent, or trade personal information submitted through this site.

We disclose information only in the following circumstances:

• Published clinic profiles: Clinic name, city, address, phone, website, treatments, and clinical data that you submit for listing purposes will be displayed publicly on the directory.
• Legal obligation: If required by applicable law, court order, or government authority.
• Service providers: Google (Sheets/Apps Script) and Cloudflare (hosting), as described in Section 4.

6. Your Rights (CCPA and US Residents)

If you are a California resident, the California Consumer Privacy Act (CCPA) provides the following rights regarding personal information we hold about you:

• Right to know: What personal information we have collected, used, disclosed, or sold
• Right to delete: Request deletion of personal information we have collected from you
• Right to opt out: We do not sell personal information. No opt-out action is required.
• Right to non-discrimination: We will not discriminate against you for exercising your privacy rights

7. Data Security

All data transmission between your browser and our servers uses HTTPS encryption. Form submissions are transmitted to Google Apps Script over HTTPS. We take reasonable technical precautions to protect your information.

No method of internet transmission or electronic storage is 100% secure. We cannot guarantee absolute security of information transmitted to or stored on our systems.

8. Children's Privacy

This website is directed at fertility clinic operators and medical professionals. It is not directed at individuals under the age of 18. We do not knowingly collect personal information from anyone under 18. If you believe we have inadvertently collected information from a minor, contact us immediately at [email protected].

9. Third-Party Links

Fertility Network USA links to external websites including the CDC (cdc.gov), SART (sart.org), and individual clinic websites. We are not responsible for the privacy practices or content of those external sites. Review the privacy policy of each external site you visit.

10. Changes to This Policy

We may update this Privacy Policy when our data practices change. The effective date at the top of this page reflects the date of the most recent revision. Continued use of the site after an update constitutes acceptance of the revised policy.
