In Vitro Fertilization (IVF)
In vitro fertilization (IVF) is an assisted reproductive technology (ART) procedure in which eggs retrieved from the ovaries are fertilized with sperm in a laboratory, and the resulting embryo is transferred to the uterus. The CDC National ART Surveillance System (NASS), 2022 Annual Report records a 49.7% live birth rate per egg retrieval for patients under 35 using their own eggs.

The IVF Process — 5 Clinical Stages
IVF proceeds in five sequential clinical stages managed by a reproductive endocrinologist and embryology laboratory. The typical cycle spans 4–6 weeks from stimulation start to pregnancy test.
Ovarian Stimulation
Injectable gonadotropins (FSH and LH) are administered daily for 8–14 days to stimulate multiple follicle development. Response is monitored via serial transvaginal ultrasound and serum estradiol. Stimulation ends when lead follicles reach 17–20mm. A trigger injection (hCG or GnRH agonist) is administered 34–36 hours before retrieval.
Egg Retrieval
Eggs are retrieved via transvaginal ultrasound-guided needle aspiration under intravenous sedation. The embryologist immediately assesses each egg for maturity in the laboratory. The procedure takes 20–30 minutes.
Fertilization
Mature eggs are fertilized using conventional insemination or ICSI (intracytoplasmic sperm injection), in which a single sperm is injected directly into each egg. Fertilization confirmation occurs 16–18 hours post-insemination.
Embryo Culture and Assessment
Fertilized embryos develop in culture for 5–6 days to the blastocyst stage. Embryologists grade each embryo morphologically. Preimplantation genetic testing (PGT-A) is performed on day 5–7 biopsy if indicated by age, recurrent loss history, or chromosomal carrier status.
Embryo Transfer
A single blastocyst is transferred to the uterus via thin catheter under ultrasound guidance. No anesthesia is required. A serum beta-hCG pregnancy test is performed 9–11 days post-transfer. Surplus embryos are vitrified for future frozen embryo transfer (FET) cycles.
IVF Success Rates — CDC NASS 2022
How the CDC Measures IVF Success
The CDC measures IVF success as the live birth rate per egg retrieval: retrievals resulting in at least one live birth ÷ total retrievals. The CDC segments this by patient age group, egg type (own vs. donor), and embryo type (fresh vs. frozen). The 2022 Annual Report covers ~847,000 ART cycles across 582 US clinics.
Live Birth Rate by Age — Own Eggs (CDC NASS 2022)
| Under 35 | 49.7% per retrieval — highest success cohort |
| 35–37 | Declines from under-35 rate |
| 38–40 | Further decline; individualized assessment required |
| 41–42 | Substantially lower; donor egg IVF often recommended |
| Over 42 | Lowest own-egg rate; donor eggs typically indicated |
| Donor egg (any age) | Outcome determined by donor's age, not recipient's |
Clinic-Level Variation
Within the 582 indexed clinics, CDC-reported live birth rates range from under 1% to 44% across all age groups combined. This variation reflects patient population mix, case complexity, and cycle volume — not direct clinic-to-clinic comparability. Always compare clinics by patient age cohort using clinic-level CDC profiles.
IVF Cost in the United States
IVF cost is not federally regulated and varies by clinic and market. No federal body publishes a national average. The ranges below reflect published clinic fee schedules and industry data.
| Base IVF cycle (monitoring + retrieval + transfer) | $12,000–$25,000 |
| Gonadotropin medications | $3,000–$7,000 per cycle |
| ICSI | $1,000–$2,500 add-on |
| PGT-A (genetic testing) | $3,000–$6,000 |
| Embryo storage (annual) | $500–$1,000 |
| Frozen embryo transfer (FET) | $3,000–$5,000 |
| Donor egg IVF | $25,000–$50,000+ |
| State insurance mandates | 21 US states mandate fertility coverage (scope varies) |
Clinical Indications for IVF
ASRM guidelines identify these as primary IVF indications:
- Bilateral tubal factor (blocked or absent fallopian tubes)
- Severe male factor infertility requiring ICSI
- Failure of 3–6 IUI cycles with unexplained infertility
- Advanced maternal age (38+) where time-sensitive treatment is indicated
- Diminished ovarian reserve
- Endometriosis with structural pelvic impact
- PGT-A requirement (chromosomal carrier, recurrent aneuploid pregnancies)
- Same-sex male couples or single individuals requiring gestational surrogacy
IVF vs IUI — Key Differences
| Fertilization location | IVF: laboratory / IUI: fallopian tube (in vivo) |
| Egg retrieval | IVF: required / IUI: not required |
| Success rate per cycle | IVF: 49.7% under 35 (CDC 2022) / IUI: 10–20% (literature) |
| Cost per cycle | IVF: $15,000–$30,000 / IUI: $300–$1,500 |
| Invasiveness | IVF: egg retrieval under sedation / IUI: office procedure |
| CDC reporting | IVF: mandatory via NASS / IUI: not tracked by CDC |
